Reinstate your licence

If you were a former pharmacist or pharmacy technician in Alberta, you may be eligible for reinstatement. Reinstatement procedures differ depending on your current situation.

Reinstatement policies

I am currently licensed as a practicing pharmacist in another Canadian province.

  • Application for reinstatement. Contact ACP for application form registrationinfo@pharmacists.ab.ca.
  • Statutory Declaration (page 3 of the application)
  • Criminal Record Check. ACP recommends the use of BackCheck for obtaining a criminal record check.  Please use the following link (http://www.backcheck.ca/pharmacists-ab/) to begin the process.  Choose the Enhanced Police Information Check option as ACP will not accept the Canadian Criminal Record Check only.
  • A letter of standing from any jurisdiction, other than Alberta, where you are currently or previously licensed. The letters must indicate your current status within that jurisdiction.
